Approaching end of life can be overwhelming for everyone involved. JSSA is here to help you and your loved ones make this transition as smoothly as possible, recognizing moments of joy and celebration along the way.
We provide support free of charge to neighbors with a life-limiting condition – including but not limited to advanced dementia, congestive heart failure, metastatic cancer, and renal failure – who are not eligible or emotionally ready for hospice.
As part of our program JSSA’s specially-trained volunteers visit weekly to provide companionship and engage your loved one in a variety of ways:
- Sharing memories
- Playing games
- Reading aloud
- Listening to music
- Looking at family photographs
- Spending time outdoors
A nurse coordinator also visits at least once a month to monitor participants’ health and well-being, provide hospice education, make referrals for additional community support, and assess participants’ homes for safety. “Home” may be a private residence, a retirement community, a nursing home, an assisted living facility, or an independent living facility.
Our caregiver support services are available for up to one year.
