Our diverse team of care managers works with older adults and their families to access/learn about available community resources and develop a plan of care based on you or your loved one’s changing needs.
Care Management is designed to provide support to older adults and their loved ones. Care management often includes regular phone calls and visits, connection to community resources, and guidance and support for older adults, caregivers, and/or family members as life situations change.
Elder Care Consultations are designed to be short-term and are focused on identifying needs, accessing resources, developing, and implementing an action plan. These consultations often focus on changes in functioning (self or loved ones), having difficult conversations with family members, navigating resources available in the community, and exploring questions about living situations.
