For: Children, adolescents and adults with special needs and their families.
Service description: A wide range of specialized and coordinated services that include: - Education about options in financial and estate planning, guardianship, and special needs trusts
- Assessment and care planning to determine current and future service needs.
- Education and advocacy on public entitlements and other community resources including supported and subsidized housing, health insurance, vocational services, and social/recreational programs.
- Assistance in documenting needs and wishes at present and for the future in a letter of intent.
- Monitoring the adequacy and appropriateness of current and future service arrangements
- Direct guidance and support
- Ongoing maintenance of relationships and historical information that help ensure continuity of care and high-quality service
- Help in achieving maximum independence through linkages and referrals to a wide range of specialized community resources
Service providers: Clinical social workers, case managers, psychologists, board-certified psychiatrists and educational specialists.
