Comprehensive In-Home and In-Office Assessments
For: Seniors, their families and caregivers.
Service description: JSSA provides comprehensive in-home or in-office assessments to determine the overall functioning capacity of the senior, his/her particular areas of need, and the services and programs needed to implement the most appropriate care plan for maintaining independence. The family, caregivers and physicians are consulted as needed.
Service providers: Clinical social workers, geriatric case managers and nurses.

Payment: Information about JSSA's fees and charges is available upon request. JSSA accepts full fees for this service. JSSA can provide financial assistance on a sliding fee scale based on need.
