Caregiver and Family Consultation and Support
For: Seniors, their adult children, families and caregivers.
Service description: Consultations before the assessment usually help to establish an appropriate course of action to address a specific caregiving issue or determine further assessment of needs. Once a needs assessment has been completed and a care coordination plan implemented, JSSA provides ongoing caregiver and family consultation and support. Services could include in-office sessions with family members or caregivers to discuss particular areas of concern, referrals to 
support groups and educational workshops on a variety of topics and links to helpful community resources.
Service providers: Clinical social workers and geriatric case managers.
