Transitions Care Transitions is a nonsectarian, nationally recognized social support program designed to help people living with life-threatening illness and their families. Specially trained volunteers visit once a week and can provide companionship, emotional support, help with errands and shopping, and respite for caregivers. The Transitions coordinator can provide home visits for support, information and linkage to local community resources. Transitions services are provided throughout Montgomery County to individuals at home, in an assisted living facility or in a nursing facility. The Transitions program is funded by community gifts, and there is no charge for services.
