Service Planning and Care Coordination
For: Children and adolescents, ages 4 through 17 and their families.
Service description: Once a comprehensive evaluation identifies a child or teen's particular areas of need, a plan of service and care coordination is created and implemented. Recommended services could include individual, group or family
workshops, a
psychiatric evaluation and/or
medication management plan, educational or neuropsychological
testing and consultation, specific
support groups or links to helpful community resources. JSSA works with you, your child, family, and involved professionals to monitor and coordinate care on an on-going basis.
Service providers: Clinical social workers, psychologists, board certified psychiatrists and educational specialists.

Payment: Information about JSSA's fees and charges is available upon request. JSSA accepts full fee as well as most major insurance and managed care plans for this service, as well as full fee for service. JSSA also accepts Medicare and Medicaid and can provide financial assistance on a sliding fee scale fee based on need.
