Service description: For more than 25 years, JSSA's hospice has provided a wide range of quality medical and support services, from symptom and pain management to emotional and spiritual guidance to families in the community:
- Admission by registered nurses
- Patient evaluation and assessment
- Seven-day-a-week admission
- Collaboration with the patient's primary physician
- Instruction in pain and symptom management
- 24-hour nursing care availability
- Care coordination and supervision
- Flexible scheduling to accommodate patient and family needs
- Frequent in-home nursing visits
- Ongoing medical consultation
- Coordination of medical equipment
- Personal care assistance
- Nutritional advice
- Emotional support and counseling
- Pastoral counseling
- Volunteer companionship and respite care
- Bereavement support
Service providers: Medical directors/physicians, highly skilled registered nurses, experienced clinical social workers, certified nursing assistants skilled in end-of-life care, pastoral counselors/chaplains and rabbis, extensively trained volunteers.
Service location: JSSA's hospice services can be provided in a patient’s home or in the home of a family member, senior living residence, assisted living facility, or nursing home and are available throughout Montgomery County.
