Transitions Care

IN THE NEWS!:  JSSA Hospice and Transitions volunteers awarded 2009 Marriott Spirit to Serve Community Service Award

For: Any individual in the community who has a serious, life-threatening illness; may be undergoing medical treatment, and is living at home, in an assisted living facility, or in a nursing facility.
 
Service description: Transitions is a free, nonsectarian, nationally recognized all-volunteer program designed to help people living with serious illness and their families cope with the challenges they face by providing companionship, emotional support, help with errands and shopping, and respite for caregivers. Transitions can also provide information and linkage to local community resources.
 
Service providers: Specially trained Transitions volunteers and a Transitions nurse coordinator who enhances the volunteer support by providing information and linkage to local community services, home visits and telephone check-in calls.
 
Service location:  Transition services are provided to individuals at home, in an assisted living facility, or in a nursing facility throughout Montgomery County.
 
Getting started: To become a Transitions patient, please call our Transitions coordinator at 301-816-2681 or send us an    email inquiry.  The Transitions coordinator will then contact the individual’s physician to make sure that Transitions support is appropriate. If you are interested in becoming a Transitions volunteer, please call  301-816-2681 or send us an  email inquiry.
 
Payment: The Transitions program is funded by community gifts and is free to participants.
