Transitions Care
Service description: Transitions is a free, nonsectarian, nationally recognized all-volunteer program designed to help people living with serious illness and their families cope with the challenges they face by providing companionship, emotional support, help with errands and shopping, and respite for caregivers. Transitions can also provide information and linkage to local community resources.
Service providers: Specially trained Transitions volunteers and a Transitions nurse coordinator who enhances the volunteer support by providing information and linkage to local community services, home visits and telephone check-in calls.
Service location: Transition services are provided to individuals at home, in an assisted living facility, or in a nursing facility throughout Montgomery County.
Getting started: To become a Transitions patient, please call our Transitions coordinator at

Payment: The Transitions program is funded by community gifts and is free to participants.
